These are the supplemental PDF forms for the online International Student Application:
- Financial Affidavit Form - required for all applicants
- Guardian Statement - for students who will be under the age of 18 when they start classes at LAPC
- Student Status Verification Form - for students who are transferring their SEVIS record to LAPC from another SEVIS approved school

Use your LACCD student email account to access the online forms below. Allow up to 3 days processing time once your form has been received by the ISS Office. Please contact the ISS Office if you have any questions or concerns.
